To ensure a safe, respectful, and professional environment, please adhere to the following rules.

1. Health, Safety, & Consent

• A valid government-issued photo ID is required for all services. I tattoo adults 18+ only, without exception.
• All clients must complete a digital or physical consent form prior to any service.
• Please inform me of any medical conditions, allergies, or medications before your appointment. Certain conditions may require a doctor’s note to proceed.
• I reserve the right to refuse service to anyone whose health or behavior presents a risk. This includes, but is not limited to:
  • Appearing to be under the influence of alcohol or drugs.
  • Having a contagious illness (cold, flu, etc.).
  • Behaving in a threatening, aggressive, or inappropriate manner.
  • Under the age of 18 years old (I will NOT tattoo a minor for any reason even with parents’ permission).

2. Booking & Deposits

• A non-refundable deposit is required to secure all appointments. This deposit comes off the final price of your session.
• Deposits are forfeited if you:
  • Cancel with less than 48 hours’ notice.
  • No-call/no-show for your appointment.
  • Arrive excessively late (typically beyond 15-30 minutes) and cannot be accommodated.
  • Arrive unprepared or unable to be tattooed (e.g., no valid ID, under the influence, sunburned skin).
• Rescheduling: You may reschedule your appointment once with at least 48 hours’ notice, and your deposit will be transferred.

3. During Your Appointment

• Guests are welcome, but please be mindful. The studio is a single-artist space, so please keep the group small (1-2 people max) and ensure they respect the working environment.
• Practice basic hygiene. Please show up clean and prepared.
• Respect the focus. Please limit phone calls and keep conversations respectful. Headphones are welcome if you need to zone out.
• My interpretation and execution of your idea are final. I am not responsible for the meaning or spelling of text or symbols you provide.

4. Pricing & Payment

• All sales are final. I have a strict NO REFUND policy on tattoos and retail sales.
• Final pricing is based on my hourly rate or the agreed-upon price for the piece. The quote given is an estimate.
• I accept cash and major credit/debit cards. A card transaction fee may apply.

5. Art & Design Ownership

• The custom design I create is my intellectual property. I do not release original design drawings without a purchased commission.
• Flash/Pre-drawn designs are available on a first-come, first-served basis and cannot be replicated without my permission.

6. Touch-Ups & Aftercare

• I stand by my work. Free touch-ups are offered within 3-6 months of your initial appointment, at my discretion, and for a fee if needed due to improper aftercare or client negligence.
• It is the client’s responsibility to follow the provided aftercare instructions meticulously to ensure proper healing. Failure to do so may void any free touch-up policy.

7. Photography

• I photograph my work for portfolio, social media, and advertising purposes. By accepting my services, you grant permission to use images of your tattoo unless you explicitly tell me not to before you leave the studio.

8. Liability

• By signing the consent form, you agree to release me (Matthew Peterson, operating as Mindset VonZantuk ) from all liability associated with the tattooing process and any potential complications, as outlined in the detailed consent agreement.
